This is a weight loss plateau β€” and it is one of the most frustrating (and most common) experiences in any weight loss journey. The good news: it is not a sign that you have failed. It is a sign that your body has adapted, and your approach needs to adapt with it.

Why Plateaus Happen β€” The Biology

When you lose weight, your body does not go quietly. It responds with a cascade of metabolic adaptations designed to defend your previous weight. Understanding these mechanisms is the first step to overcoming them.

The GLP-1 Plateau: What Is Different

Patients on semaglutide or tirzepatide often experience an initial period of rapid progress followed by a slowdown around months three to six. This is not a medication failure β€” it reflects the same metabolic adaptation that happens with any weight loss approach, layered on top of how GLP-1 receptor agonists work.

7 Evidence-Based Strategies to Break Through

1. Reassess Your Caloric Target

As you lose weight, your total daily energy expenditure decreases. A calorie target that produced a deficit at 200 lbs may be at maintenance at 175 lbs. Recalculating your needs β€” ideally with a provider who can review your labs and body composition β€” is often the single most effective step.

2. Prioritize Protein at Every Meal

Protein has the highest thermic effect of any macronutrient β€” your body burns more calories digesting it. It also preserves lean muscle mass during a caloric deficit, which protects your metabolic rate. Aim for 0.7–1.0 grams of protein per pound of goal body weight daily.

3. Add or Adjust Resistance Training

Cardio burns calories during the session; resistance training builds muscle that burns calories around the clock. If you have been doing primarily cardio, adding two to three strength sessions per week can restart progress within weeks.

4. Consider a Dose Adjustment or Medication Switch

If you are on a GLP-1 program and have plateaued, a dose titration or switch from semaglutide to tirzepatide (which targets both GLP-1 and GIP receptors) may reignite progress. This is a clinical decision β€” not something to self-manage β€” and requires a provider review.

5. Add Lipotropic Injections
